This guide (v.KT.2018.01.12) has been customized for use of Ages & Stages Questionnaires®, Third Edition (ASQ-3™) and Ages & Stages Questionnaires®: Social-Emotional, Second Edition (ASQ:SE-2™) as a kindergarten readiness “snapshot” under the Kansas Department of Education’s Kindergarten Readiness initiative. It is intended to support Kansas teachers/providers using ASQ Online to collect kindergarten entry data. Use outside this purpose is not permitted, and the contents of this guide may not be photocopied or disseminated further without explicit written permission.

I. GETTING STARTED
Logging in to ASQ Online
Your Program Administrator will create a user account that opens access to ASQ Online. You will receive an email with your username and steps to follow to create a password.
- To log in, click the "activate your account" link from the email message.
- Create your password by entering it twice on the Reset Your Password page.
If you receive an error message that says your password creation link has expired, follow the hyperlink to reset it again.
ASQ Online Overview
When you log in, you will be directed to the ASQ Online home page. The main sections of ASQ Online are labeled on the main navigation menu:
- Home: Access Provider Snapshot, Announcements, Resource Corner, Quick Links
- My Profile: Update your profile and set alerts
- Program: View program profile and search for users
- Child Profiles: Manage profiles and enter questionnaire data
- Screening Management: Manage tasks and print documents
- Reports: Generate reports for children and classrooms
- Family Access: Share access link with parents/caregivers
Creating Child & Parent/Caregiver Profiles
Your Program Administrator may have imported child and parent/caregiver data or manually created the profiles assigned to you.
If instructed to create your own profiles, follow these steps:
- From the main menu, select "Child Profiles."
- Select "Add Child Profile."
- Enter the required information in the Child Profile.
- Click "Save."
To enter a new Caregiver Profile:
- Select "Add New Caregiver."
- Enter the parent’s/caregiver’s information.
- Confirm the primary caregiver.
- Click "Save."
II. PREPARING FOR QUESTIONNAIRE COMPLETION
Introducing ASQ-3 and ASQ:SE-2 to Parents/Caregivers
- Share the purpose of screening: ASQ-3 provides a quick check of your child’s development.
- Provide guidance to parents for completing the screening successfully.
Providing Questionnaires to Parents/Caregivers
ASQ Online offers resources to help introduce parents/caregivers to ASQ-3 and ASQ:SE-2. You can print blank copies of these resources.
Printing Program Documents and Letters
Your Program Administrator may create custom letters/documents in ASQ Online. Access these documents under "View/Print Documents" from Screening Management.
Creating Your Own Documents and Letters
You can create custom letters and documents in ASQ Online. Click "Create Document" to begin.
Ways to Provide Questionnaires
Questionnaires can be completed online or on paper, independently or with assistance. Send questionnaires electronically or use printed copies for parents/caregivers to complete.
III. ENTERING QUESTIONNAIRE DATA INTO ASQ ONLINE
Entering Questionnaire Results Using Long or Short Form
- From "Child Profiles," find the child's name.
- Select "Add Screening."
- Enter questionnaire results using the long form or short form.
- Click "Save".
Entering Questionnaire Results via Family Access
Parents/caregivers can enter questionnaire results through Family Access. They will enter their demographic information and screening results after completing the questionnaire.
Linked ASQ-3 and ASQ:SE-2 Family Access Pages
Your program administrator can link the access pages for parents/caregivers.
Reviewing Questionnaires from Family Access
Once a questionnaire is completed, your program administrator will review and assign it to you.
IV. INTERPRETING RESULTS AND NEXT STEPS
Information Summary
The Information Summary sheet provides a summary of questionnaire results for each child, including total scores and recommendations for follow-up.
Printing Completed Questionnaires and Information Summary Sheets
You have the option to print completed questionnaires and summaries as needed.
Printing Completed Item Response Sheets for ASQ:SE-2
Select "View/Print Packets" to print completed item response sheets.
Differences Between ASQ-3 and ASQ:SE-2
Interpreting results differs between ASQ-3 (below cutoff indicates concern) and ASQ:SE-2 (above cutoff indicates concern).
