Steps for Teachers Using Together with Parents

  1. Select the appropriate questionnaire for each child in the classroom.

  2. Be sure to determine the exact age and date of birth so the proper interval is selected.
    Begin to administer the ASQ3.
    Observe the children over time and create opportunities to demonstrate skills. Leave items blank that you do not have an opportunity to observe.

  3. Once the ASQ3 has been administered, meet or call with parents.
    Ideally, parents should come to a parent–teacher meeting with a completed or partially completed questionnaire.

  4. If your observations differ from that of the parent, then determine with the parent how to ultimately mark the item, or mark the item as sometimes.
    Parents should have the final say as to how an item is scored. Be sure to discuss any parental concerns regarding the Overall section.
    Consider obtaining a signature on the completed questionnaire to document parent participation.

  5. Provide parents with follow up activities and resources.

  6. One way to ensure that parents are included in the ASQ3 process is to have parents sign a statement such as the following:
    "My input was gathered to complete the ASQ3, and I believe the information is accurate."

  7. Score the ASQ3 and discuss referral considerations if necessary.
    Discuss the scoring process and review the scores with parents. Together, make decisions about next steps.
