The Statewide Coordinated Developmental Screening System allows sharing of information so anyone who works with your child can, with your consent, have a better understanding of your child’s growth and development. This means we can all celebrate and develop your child’s strengths, while offering extra support in areas where your child might need it.
How can I support my child’s development?
Fill out an Ages & Stages Questionnaire. The agency that gave you the questionnaire will contact you with results.
What if I already completed this questionnaire in the past?
The Ages & Stages Questionnaire is designed to be repeated at intervals throughout your child’s first years, in much the same way as well-child visits are.
What if my child has an area of concern?
The questionnaire is a screening tool, it does not provide a diagnosis. Your ASQ is not a test, but rather a screener to check how your child is developing. You will answer if your child can complete different activities: “yes,” “sometimes” or “not yet.” You will also have the opportunity to share any concerns about your child. The questionnaire takes about 10–15 minutes to complete.

FAQs
Choosing “yes” allows your data to be shared, and your child’s screening results are available to treating pediatricians via the electronic medical records system. Other providers to whom you give permission may also view this information. Data will be shared for research, but without your child’s identifying information.
