This workflow map outlines the steps involved in a screening program for a home visiting agency and who is responsible for each step.

Steps of the Workflow

  1. Distribute ASQ®:SE-2 questionnaire
    AS alerts HV 1 month prior to upcoming ASQ:SE-2 target date. AS prints appropriate ASQ:SE-2 interval with child’s name and ASQ:SE-2 target date; AS passes to HV.

  2. Complete ASQ:SE-2
    HV provides ASQ:SE-2 to parent 1 week prior to ASQ:SE-2 target date. Parent completes ASQ:SE-2 prior to ASQ:SE-2 target date (with HV support).

  3. Score ASQ:SE-2
    HV scores ASQ:SE-2 during home visit with parent. or HV scores ASQ:SE-2 immediately after home visit if score is expected to be close to or above cutoff score.

  4. Score is above cutoff / Score is close to cutoff / Score is or parent concerns well below cutoff
    HV discusses ASQ:SE-2 results with family within 1 week. HV (and supervisor, when needed) communicates screening results with family. HV provides copy of ASQ:SE-2 Information Summary to AS. HV and parent determine appropriate follow-up (e.g., provide parent–child activities, targeted information on behavior of concern, make referrals).

  5. Referrals / No referral / Screening summary and follow-up
    Parent and HV make referral(s) for a social-emotional support. HV provides in-house follow-up as indicated. AS records ASQ:SE-2 score. AS schedules child for next screening. AS records ASQ:SE-2 score. AS schedules child for re-screening.

  6. Eligible for social-emotional support services / Not eligible for social-emotional support services
    • AS – administrative staff
    HV alerts AS regarding eligibility and removal from screening program. HV provides in-house follow-up as indicated. AS records ASQ:SE-2 scores. AS schedules child for re-screening.
    • EI/ECSE – early intervention/early childhood special education
    • HV – home visitor(s)
