Score Adjustment Chart for ASQ-3™
For Use When Item Responses are Missing
When one or two items in an area are unscored, area item scores are averaged and the total area score is adjusted. Adjusted scores ensure the child is not penalized for unanswered items or items that are inappropriate given their cultural context. An area should not be scored if more than two items are unanswered.
The table below lists the adjusted area scores. Score the completed items on the questionnaire, and find the area score in the left-hand column below. Follow across to the right to determine the adjusted area score if one or two items are omitted.

Example
A child’s total score for answered items in an area is 20 points, but one item is omitted. The child’s adjusted area score will be 24 points.
