Where do I enter keycodes?

Keycodes can be entered by either the Program or Account Administrator. If a Program Administrator is adding a keycode, they will go to the "Program" tab and scroll half-way down the page to the "Keycodes" section. There, they will click "Add" and enter the keycode of the product they want unlocked for use in ASQ Online.

What if my ASQ Keycode is not working?

Do paper copies of the questionnaires need to be purchased for usage of the online system?

Yes, in order to use ASQ Online, the online management and online questionnaire completion system for ASQ-3 and ASQ:SE-2, a program must first purchase print copies of the questionnaires that the program wishes to use in the online system. If a program wishes to use ASQ-3 in both English and Vietnamese, the program must purchase both versions.

How can screening data for the ASQ-3 translations (Arabic, Chinese, Vietnamese, and French) be entered into ASQ Online?

The translations commercially available from Brookes all work just like English and Spanish questionnaires in ASQ Online. The CD-ROMs have keycodes that unlock the translated questionnaires for entering results, printing, and setting up Family Access pages. Keycodes were not included on the initial printings of ASQ-3 questionnaires.

Can keycodes be shared with other sites?

No, keycodes may not be shared between sites. Each keycode may be used only once. After a keycode has been entered in the ASQ Online system, it will not unlock questionnaires for another account or program.

Where do I find the ASQ keycode?

The ASQ keycode is printed on a sticker that is affixed to the inside of your box of ASQ-3 or ASQ:SE-2 questionnaires or inside the front cover of your Learning Activities book. Keycode stickers are different colors on different products:

Does the keycode give me access to all ASQ questionnaires online?

An ASQ keycode grants online access only to the type of questionnaires it came with. The keycode that appears on a box of ASQ-3 questionnaires in English will allow access to ASQ-3 English questionnaires within your ASQ Online account. The keycode that appears on a box of ASQ:SE-2 questionnaires in Spanish will allow access to ASQ:SE-2 Spanish questionnaires.

What if my ASQ keycode is missing or lost?

If you lose your ASQ keycode, contact Brookes Publishing via the Customer Support Center or by phone (1-800-638-3775). If you are an international customer, contact information can be found on their website. They will email you a Keycode Replacement Request Form. You can also download the Keycode Replacement Request Form from the support website.

How many keycodes do I need?

If you are a single-site organization purchasing an ASQ Pro subscription, you need one keycode for each type of questionnaire you are interested in using in the online system. For example, if your organization would like to use ASQ-3 in English and ASQ:SE-2 in English, you will need one ASQ-3 in English keycode and one ASQ:SE-2 in English keycode.

Once your ASQ Online subscription is activated, the keycode can be entered into the system. The keycode is entered within the Program Details section and can be entered by the account administrator or the program administrator.
