# My care provider asked me to complete ASQ-3. Is there a privacy policy for the questionnaire? Is the information protected by HIPAA?

If you are completing an online version of ASQ-3, please [read the privacy policy](https://www.asqonline.com/legal/privacypolicy.pdf). As a parent/caregiver of a child, you are considered an ASQ Family Access Authorized User; your care provider is considered the Subscriber. The end of the policy has information about the security of the ASQ Online website.

If you are completing a paper version of the ASQ-3, you should ask your care provider about their privacy practices. Whether completed online or paper, your child's information is protected by the Health Insurance Portability and Accountability Act of 1996 (HIPAA).
