Is a consent form mandatory? Why would one get a consent form?

The ASQ developers recommend that programs obtain the written consent of parents or caregivers when screening a child. A signed consent form indicates that the parent understands the purpose of the screening program and is agreeing for their child to participate. Programs can also use the consent form to obtain permission to share ASQ results with the local education agency or a pediatrician, if necessary. The consent forms in the User’s Guides are only examples; programs may create their own consent forms. It is important to note that programs should adhere to any state, district, or program regulations regarding parental consent and sharing of information.
